Dependent Verification Worksheet (pdf): If you are a dependent student as defined by the Department of Education, the Dependent Verification Worksheet must be completed and signed by the student and his or her parent. Please return to the Saint Mary's College Financial Aid Office with signed copies of the 2008 Federal tax returns for the custodial parent(s) and student, including all schedules and W-2's.
Independent Verification Worksheet (pdf): If you are an independent student as defined by the Department of Education, the Independent Verification Worksheet must be completed and signed by the student. Please return to the Saint Mary's College Financial Aid Office with a signed copy of the 2008 Federal tax return for the student and spouse (if the student has a spouse), including all schedules and W-2's.
Student/Parent Non-Filer Form (pdf): If the student and/or the parents were not required to complete a tax return for 2008, please complete the appropriate section of the form, sign, and return to the Financial Aid Office.
Additional Financial Information (Dependent) (pdf): These worksheets record untaxed income or credits which may have been received in 2008 for Dependent students and their parents. Please complete the sections(s) required, sign, and return to the Financial Aid Office.
