ID Numbers and Photo ID Cards
ID Numbers
Employee ID numbers are available from the Human Resources Office (Filippi Hall), Mon - Fri, 8:30 am-4:30 pm. Fill out required I-9 and W-4 forms as well as the Personnel Data Sheet, and present required "acceptable" legal documentation (ie, one current passport, or both driver's license and social security card; or for complete instructions view the last page of the I-9 form for List of Acceptable Documents).
Employment Eligibility Verification, I-9 Form.
Call the HR office, ext. 4212, a few days later for your ID number.
Photo ID Cards
Photo ID cards are issued by the Business Office (Filippi Hall), Mon - Fri, 9:00 am-3:30 pm; no appointment necessary. Bring your SMC ID number.
