1. The Faculty RSVP and Regalia Form is for all faculty to fill out in order to RSVP to the Undergraduate and Graduate and Professional Studies Commencement Ceremonies, as well as to the Faculty/VIP Commencement Luncheon and the Faculty Breakfast. This form also allows you to rent regalia through the College if you don't have your own.

**Note: If you wish to attend Commencement, you MUST fill out this form. Your RSVP is necessary so that we can reserve a seat for you on stage and make sure we have enough regalia for every borrower

2. The Faculty/Staff Presenting Diplomas to Relatives Form is for faculty and staff who have a daughter, son, husband, wife, granddaughter, grandson, sister or brother graduating to whom they would like to present a diploma.

In order to present a diploma to your relative during Commencement, you need to fill out this form.

3. The Departmental Academic Awards Information Form is for department heads and thier staff, in order to input the names of their Departmental Award Recipients directly to the Commencement Office for the ordering of plaques.
