How to report a suspected violation of the graduate and professional academic honor code:

  1. Meet with student(s).
  2. If you still suspect a violation of the acadedemic honor code is possible, complete the Faculty-Student Agreement form, and sign it along with student.
  3. Submit contract, assignment in question, course syllabus, and brief written narrative explaining what led to this report to the GPAHC Coordinator, in FAH 250.
  4. Submit confidential Report of Suspected Academic Dishonesty form online to the GPAHC: Report form.
  5. The coordinator will contact the student(s) being reported and arrange a confidential meeting to discuss what this process entails, what their options are, and offer additional emotional support and the appropriate campus referals as needed.
