To see if you qualify read the information below and fill out the H1N1 Flu mist form.
- Make sure you can receive the H1N1 Vaccine by reading the H1N1 Flu Vaccine Information Sheet.
- Print and fill out the H1N1 Flu Form and bring it to the Health and Wellness Center.
