Registration Requirements
All new students are required to submit a complete Medical Evaluation Form before they begin classes at Saint Mary’s College. This form consists of personal health history information, up to date immunization information and a physical exam within the last 12 months. If the completed form is not submitted on time, it could result in registration difficulties. Medical Evaluation Form (pdf)
Required immunizations:
- Tetanus: within 10 years
- MMR: 2 doses required
- PPD:(tuberculosis testing) for international and high-risk students, please consult your private physician
- Varicella (chicken pox): history of disease or 2 doses required
- Hepatitis B: 3 doses required
Recommended immunizations:
- HPV: 3 doses (females only)
- Hepatitis A: 2 doses
- Meningococcal vaccine: 1 dose
