Medical Evaluation Form
All undergraduate students enrolled at SMC are required to submit a fully completed Medical Evaluation Form to the Health & Wellness Center by August 15 for fall semester admits. For Jan Term and Spring, forms should be submitted by January 5 and February 1 respectively. The physical examination should be performed and documented by a medical practitioner no more than 6 months prior to enrollment at the College. Your form must include up-to-date immunization information as indicated. No substitute form will be accepted.
