Below are the documents that will need to be turned in prior to your departure

Pre-Departure Documents

  • Photocopy of Passport (if renewing)
  • Photocopy of Student Visa (if required by host country)
  • Copy of Flight Itinerary (electronic version OK)
  • Transfer Credit Approval Form
  • Copy of Health Insurance Card/Document
  • One picture of yourself (JPEG)
  • Signed Copy of Student Handbook (sign and submit last page of the handbook)

Please bring all completed materials to The Center for International Programs
