Pre-Departure Documents

• Photocopy of Passport (if renewing)
• Photocopy of Student Visa (if required by host country)
• Copy of Flight Itinerary (electronic version OK)
• Academic Planning Worksheet
• Copy of Health Insurance Card/Document
• One picture of yourself (JPEG)
• Signed Copy of Student Handbook (sign and submit last page of the handbook)

Please bring all completed materials to The Center for International Programs

Deadline: Friday, May 24, 2013

NOTE: We understand that some of these documents may not be available by May 24, 2013 (ex: copy of student visa and/or flight itinerary).  Simply submit the documents by the deadline, and submit the rest prior to departure.
