Call the Health & Wellness Center TODAY at 631-4254 to make your SMC Wellness Fair flu shot appointment
SMC Nurses will give 50 FREE flu shots to undergraduate on-campus residents at the Augustine Hall entrance across from the Library. Flu shots promote community immunity against influenza
BEFORE YOUR APPOINTMENT:
1) Fill out the Screening Checklist - Flu Shot Form in your Health Portal
2) Fill out the COVID-19 Screening Form in your Health Portal - if you develop any COVID symptoms, please call us to reschedule your appointment
3) Fill out the Patient Treatment Agreement Form (on or after 8/24/2020) in your Health Portal
4) Read the Influenza Vaccine Information Statement
Please bring your Student ID to your appointment. We look forward to seeing you!
