Test Kit Authorization Form

This form is intended for instructors and faculty members. In order to be eligible to borrow from the Test Kit Collection, students must be currently enrolled in a course that requires the use of test kits. Each semester, you may submit the form below for each class section that needs access to the Test Kit Collection. If a student adds your class after you have submitted the roster, please email the education librarian Conrad Woxland at cmw22@stmarys-ca.edu directly to ensure the student is added promptly
