College Incident & Concern Reporting Forms
Saint Mary's College is committed to supporting all students, employees, visitors, guests, and other third parties who may experience behavior that is antithetical to the College's core values and Lasallian values. This includes first-hand accounts of misconduct, witnessing situations that cause concern for an individuals and/or the community. To facilitate this below are the Report Forms available to direct concerns to the appropriate office(s) and staff to review and respond to the situations.
Reports are typically reviewed within 24-48 hours of submission - these forms should not be used for emergencies or requests for urgent needs. For emergencies and/or immediate response, please contact Campus Safety at (925) 631-4282 or 9-1-1.
If you are unsure which is the appropriate form to submit your information, please select the most applicable based on the description of each category - once received, staff reviewing the report will ensure it is routed to the appropriate office/person for follow-up.
Use this form if you experience, witness, or are made aware of a bias incident. A bias incident is any conduct, speech, or expression, motivated in whole or in part by bias or prejudice meant to intimidate, demean, mock, degrade, marginalize, or threaten individuals or groups based on that individual or group's actual or perceived identities.
Concerned about someone's (or your own) well-being? Use this CARES referral form to share your concerns - which might be about academics, mental or physical health, worrying behavior, or general wellness. A staff member will review the report, provide outreach, and when needed referral to appropriate resources.
Use this form for situations or concerns that fall under Saint Mary’s College’s Sex-Based Discrimination and Harassment, Sexual Misconduct, and Retaliation Policy, including sexual assault, dating and domestic violence, and stalking. May involve students and/or employees and/or third parties.
For incidents involving Intentional, knowing, or reckless act(s) that are committed in connection with (a) initiation into, (b) an affiliation with, or (c) continued membership in a group-affiliated activity, which causes or endangers physical or psychological harm and/or unreasonably interferes with a person’s successful pursuit of an academic program or institutional employment.
